Before the thread moves on from state-level differences — what is the sample size behind the claim? I am not being difficult; I have seen the same figure quoted from an n of four and from an n of four hundred.

The question about state-level differences that I actually want answered is the second one below. The first is context and I have kept it short.
Both are stated with units, and I have said what I already checked so that nobody repeats it.
Filing a mild objection to the consensus on state-level differences. Mild because I might be wrong; an objection because nobody has addressed the case that does not fit.
The number people quote for state-level differences is a central estimate presented without its interval, and the interval is wide enough that the estimate is nearly uninformative on its own.

I am describing what is, rather than arguing for what should be.
On post #4 — agreed on the reasoning, with one qualification.
Where I have landed on state-level differences, having got it wrong once in public: the direction is clear, the magnitude is not, and anyone quoting a precise magnitude has borrowed it from somewhere that did not measure it.
This follows post #7 rather than contradicting it.
One caution on state-level differences: everything above assumes the underlying documentation is what it claims to be. That assumption is doing real work and is rarely stated.
I read post #9 twice before replying, because I had assumed the opposite.
Pharmacy practice varies between pharmacies within the same city, so two honest accounts can be entirely different.
Worth separating state-level differences as a question about the compound from state-level differences as a question about the documentation. They get answered by different people and only one of them is answerable here.
